If you’ve dealt with stubborn bad breath, you’ve probably wondered whether a probiotic lozenge or a special mouthrinse is worth the money. A new randomized trial put both to the test against plain hygiene advice — and found real, but uneven, results. Zinc came out ahead.
What the research found
The study, published in the journal Microorganisms in April 2026, was a four-week, real-world (“pragmatic”) randomized trial in 117 adults who had been bothered by bad breath for at least three months and had a confirmed odor score at the start. Participants were split evenly into three groups:
- Standard care — standardized counseling on brushing, flossing, and tongue cleaning.
- Probiotic lozenges containing Streptococcus salivarius K12, a strain studied for crowding out odor-producing bacteria.
- A zinc-containing mouthrinse, which binds and neutralizes the smelly gases directly.
Researchers tracked two things: the concentration of volatile sulfur compounds (VSCs) — the sulfur gases behind most oral malodor — measured with a portable breath analyzer, and an organoleptic score, where a trained assessor rates the actual smell.
All three groups improved over the four weeks, but by clearly different amounts, and the pattern was consistent across both measures. For the sulfur gases, the average drop was about −12.7 parts per billion (ppb) with standard care, −47.3 ppb with the probiotic lozenges, and −78.5 ppb with the zinc mouthrinse (p < 0.001). The smell scores followed the same order, falling by roughly −0.2, −0.8, and −1.2 points respectively (p < 0.001). In plain terms: advice alone helped a little, the probiotic helped more, and the zinc rinse helped most.
What it means for your breath
The headline is encouraging: for persistent bad breath that isn’t going away on its own, both a zinc rinse and a K12 probiotic did measurably better than hygiene advice by itself, with the zinc rinse leading. That fits what’s long been understood about mouthwash — zinc ions chemically grab the sulfur gases and blunt the odor — while adding fresh, head-to-head evidence for oral probiotics, which have had a more mixed track record.
A few caveats keep this in perspective. This was a single, short (four-week) study, and because the three treatments look and feel completely different, participants knew which one they were using — so expectation could color the results, especially the smell ratings. “Pragmatic” also means it tested these products the way real people would use them, not under tightly controlled lab conditions. And an odor that eases over four weeks is not the same as a cure: none of these approaches removes the underlying cause.
That last point matters most. VSCs are mainly produced by bacteria on the back of the tongue and in gum pockets, so the durable fixes are still the basics — clean your tongue, brush and floss to control plaque, stay hydrated, and see a dentist to rule out gum disease. A zinc rinse or a K12 lozenge is best thought of as a helpful add-on to that routine, not a replacement for it.
